Making an Appointment
SCHEDULING:
To schedule an appointment, a patient/client should call the Office Manager, Lisa Casale, at 508-877-0882. At the time of scheduling your first appointment, you may request a Patient Packet to be sent to you (soon to be available online). Lisa Casale is also available to answer any questions you may have. Please bring this information with you to the first appointment.Office location:

CANCELLATION POLICY:
As a professional in a business that depends on scheduled appointments and the fact that I often have a long waiting list, a cancellation policy exists. Kindly call 24 hours in advance to cancel or reschedule your appointment or there will be a $50.00 fee, which is payable prior to the next session.In the case of an emergency, and a client or health-care professional needs to reach me, please call 781-237-9016.
NUTRITION COUNSELING AND INSURANCE COVERAGE:
Many insurance companies cover nutrition services for medical necessity. This may also include weight management in many cases. I am contracted with many insurance companies as a provider; however, each insurance plan carries its own benefits. It is important that you know your coverage with regard to nutrition counseling.Questions to ask your insurance company:
Does my insurance policy have nutrition counseling benefits that will cover my diagnosis?
Is a referral required by a Primary Care Physician (PCP)?
If I am not contracted with your insurance, please ask your insurance company:
Does my policy have out-of-network nutrition counseling benefits
Will I need preauthorization from insurance to see an out-of-network provider for nutrition counseling
In the case that insurance does not cover, you are welcome to schedule an appointment and a detailed receipt will be provided to you, upon request, for your submission to insurance if you wish to pursue reimbursement. However, payment for the nutrition session is due at the time of service.
If your insurance does cover for nutrition services, you must do the following:
- Determine if your insurance plan requires a referral. Please see Referrals.
- Bring your insurance card with you to the initial visit
- Plan to pay for the copay at the time of the visit
Please be advised that the following insurance plans require an insurance referral / authorization for nutrition counseling services.
- HMO – Health Maintenance Organization
- POS - Point of Service
- EPO - Exclusive Provider Option
- Medicare Preferred – Secure Horizons
*If nutrition counseling services are being provided for a diagnosis of diabetes – no referral is required.
Pre-diabetes and all other medical conditions require a BCBS authorization under the insurance plans
with the following ID# prefixes:
- XXH *
- MTN *
- MTP *
- XXC
- Please contact your primary care physician to request that an insurance referral be processed for your scheduled nutrition counseling appointments.
- If this step has been taken, and you do not have a copy of the referral / authorization, please continue to follow-up with the referral department at your doctor’s office to ensure that a referral is in place.
- Please have the processed referral faxed to 508-877-0046.
- Coverage for Nutrition Counseling services will be denied if your insurance plan does not have a referral on file. Payment will be your responsibility.
- If your insurance plan is not listed above, you have questions regarding your insurance coverage, or questions regarding this notice, do not hesitate to contact me.
