Making lifestyle changes can be difficult and often requires expert guidance and coaching along each step of the process. I will provide the advice and support needed to help you create the reality you long for. I will complete a comprehensive nutritional, medical, lifestyle and psychosocial assessment and will provide feedback and a summary to primary care physicians and other providers if desired.
You will be pleased with the attention and care given to your personal nutrition and lifestyle needs. The following pages contain information to be reviewed and completed prior to the initial session. This will facilitate our meeting, allowing us to focus on YOUR needs and not the paperwork.
