Individual/Group Sessions

Individual and group sessions are available, as well as workshops, grocery tours and online diet assessments.

• Initial session

  
  This session involves a thorough assessment of nutritional, medical, lifestyle and psychosocial history. Initial goals and strategies will be outlined. A note may be sent to your referring provider and other providers if desired.
  $140 – in office
  $175 – in home (within 7 mile radius or mileage charge included)*
  *If insurance applies, the session will be billed to your insurance company and you will be responsible for travel time.
  $85 per hour.

• Follow Up

  
  Follow up sessions are typically 25-30 minutes in duration. Assessment of progress will be discussed and revisions made if necessary.
  $85 – in office
  $125 – in home

• Block Sessions- offered to self-pay clients only

  
  A package of four follow up sessions offered at a discount rate.
  $300 – in office

• Phone Consult

  
  On occasion, phone consultations may be done, especially in cases where frequent follow up is desired, but is not feasible with regard to time. The appropriateness of such consults may be discussed during an initial or follow up meeting.
  Health insurance will not cover for this type of consultation, thus a fee for service applies.
  $85 – up to 30 minutes
  $42.50 – up to 15 minutes

• Email Check In

  
  Between session support greatly enhances the positive outcome of behavior change. Email provides an opportunity for questions to be answered, food records to be reviewed or supplement research inquiries to be done on your behalf.
  $15

• Online Assessment

  
  A detailed assessment of body weight, activity level and 3-day food recall will be done with accompanying recommendations for suggested improvements. This can be done in the convenience of your own home and at your convenience. Please be reminded that this does not take the place of a physical exam by your doctor, nor is it a medical nutrition therapy prescription.
  $175 pre-paid

• Menu Preparation

  
  A 2 week cycle menu including meals and snacks based on individual preferences.

Grocery Tours/Restaurant Meeting

Field Trip
A session to the market, health food store or restaurant, may be included as an adjunct to weekly sessions. Paid at time of visit.

Supplement Assessment/Suggestions

Supplement-Medication Query
An independent web review of your supplement – medication interactions, or simply the effectiveness and safety of your current regimen, may be done.
